MASSACHUSETTS ADDENDUM
I acknowledge that it is unlawful in Massachusetts to require or administer a lie detector test as a condition of employment or continued employment. An employer who violates this law shall be subject to criminal penalties and civil liability.
ILLINOIS ADDENDUM
